Appointment Reminders

Patients Not Attending

During May 2015, 77 patients failed to attend a prebooked appointment with one of the doctors wasting 13hrs 20mins of surgery time.

61 patients didn't attend prebooked nurse appointments wasting 16hrs 20mins of surgery time and 18 patients didn't attend with the attached teams (physio etc) wasting 7hrs 35mins.

Repeated missed appointment will result in the patient being removed from the practice list and will need to register at another surgery.

Electronic Prescribing

In May 2015,the practice and its pharmacies went live on a system called Electronic Prescribing. Initially this will be used just for patients who have regular repeat medications. This system has the benefit of a greater security between the practice and the pharmacy. As a practice, we encourage all our patients to have a conversation with their pharmacy about the scheme and make a nomination of receiving pharmacy as soon as possible. Please note patients who already have a chemist receiving their prescriptions still need to have this conversation at the pharmacy.

Low Moor Medical Practice Needs You

to become a Practice Health Champion!

Are you interested in health and would like to encourage others to improve their health and wellbeing? Practice Health Champions volunteer with their GP Practice to deliver health and wellbeing activities to patients and the wider community. To become a Practice Health Champion you will need to attend a 2 day training course which will develop your knowledge, skills, experience and passion. Following this the Practice Health Champions will volunteer with the practice for approximately 3 hours per week. The Practice Health Champions will meet regularly with GP practice staff to identify and explore health concerns and issues which affect patients of the practice.

Low Moor Medical Practice offers a high standard of care from early morning to late evening for patients attending with a wide variety of undifferentiated conditions. The Practice is staffed by our team of 6 GP and 3 experienced nurses with additional support from other clinical and non-clinical members of staff.

We are looking for an Advanced Clinical Practitioner who is forward thinking, comfortable working autonomously and has a passion for delivering advanced care. You will act as a role model by promoting clinical effectiveness and delivering quality, viable and safe services.

The role

As one of our Advanced Nurse Practitioner’s (ANP), you will use your specialist knowledge and skills to provide healthcare, independently, to patients (adults and children) attending Low Moor Medical Practice.

You will be accountable for the total episode of care within Low Moor Medical Practice; for patients with undifferentiated and undiagnosed needs.

You will act as a role model, delivering advanced practice and promoting clinical effectiveness in care. You will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, diagnosis, prescribing, health promotion and safe discharge of patients.

Our ANP’s work closely with all members of the multidisciplinary team and will play an integral role in the operational/strategic development of the service.

You will promote and facilitate evidence based practice and clinical audit and research within the service.

You will work as an autonomous practitioner within Low Moor Medical Practice delivering all aspects of management and care to a wide range of patients with varying needs. The role will work in conjunction with multidisciplinary teams to provide holistic care and medical treatment to all Low Moor Medical Practice patients.

You will either be an individual with a recognised Master’s degree qualification in Advanced

Senior Nursing Assistant required 30 - 35 hours per week

The Practice has a vacancy for a Nursing Assistant. The ideal candidate would have prior experience in General Practice. We use SystmOne clinical system and associated knowledge is desirable.


Salary Band dependant on qualifications and experience, initially 27 (pro rata) days annual leave + 8 statutory holidays.
