Appointment Reminders

Would you like a reminder of your appointment time and date sending to your mobile phone? If so please contact the reception team on 01274 697600.

Patients Not Attending

During May 2015, 77 patients failed to attend a prebooked appointment with one of the doctors wasting 13hrs 20mins of surgery time.

61 patients didn't attend prebooked nurse appointments wasting 16hrs 20mins of surgery time and 18 patients didn't attend with the attached teams (physio etc) wasting 7hrs 35mins.

Repeated missed appointment will result in the patient being removed from the practice list and will need to register at another surgery.

Electronic Prescribing

In May 2015,the practice and its pharmacies went live on a system called Electronic Prescribing. Initially this will be used just for patients who have regular repeat medications. This system has the benefit of a greater security between the practice and the pharmacy. As a practice, we encourage all our patients to have a conversation with their pharmacy about the scheme and make a nomination of receiving pharmacy as soon as possible. Please note patients who already have a chemist receiving their prescriptions still need to have this conversation at the pharmacy.

Low Moor Medical Practice Needs You

to become a Practice Health Champion!

Are you interested in health and would like to encourage others to improve their health and wellbeing? Practice Health Champions volunteer with their GP Practice to deliver health and wellbeing activities to patients and the wider community. To become a Practice Health Champion you will need to attend a 2 day training course which will develop your knowledge, skills, experience and passion. Following this the Practice Health Champions will volunteer with the practice for approximately 3 hours per week. The Practice Health Champions will meet regularly with GP practice staff to identify and explore health concerns and issues which affect patients of the practice.

Care Navigation

The right care from the right person, first time

When you ring to book an appointment, you might notice our reception team will ask

you for a brief outline of how we can help you.

This is because we have introduced something called Care Navigation. It means our

reception team has been trained to ensure they can help you to find the best service for your

needs, so you can receive the right care more quickly. Anything you share with our team will

be kept confidential.

Through this specialist training, our team will be able to direct you to the most appropriate

clinician or service for your needs.

Our reception team will never offer clinical advice. Instead, this new way of working is about

offering you the choice to see more appropriate professionals in our practice team or even

somewhere else. If they can deal with the problem directly, it will often be quicker and means

you may not need to see a GP at all.

By working this way, it helps to free-up time for GPs to care for patients with complex or

serious health conditions that can only be managed by a doctor. More importantly though, it

means people are seen first by the clinician that is best placed to help you with your health

needs.

 

Helping you to choose

The choice is always yours though. It is completely up to you if you want to share

information with our reception team.

You will never be refused a GP appointment but we hope next time you contact our practice

and speak with our reception team you will see the value of seeing another healthcare

professional if they are able to help.

BREAKING NEWS

The practice is no longer able to accept incoming faxes. Please send you messages by email or in person in the practice. Please do not order repeat prescriptions by email as this is only monitored for a limited time each day.

URGENT NEWS about Flu vaccines.

We now have flu vaccinations in for all those who are diabetic, suffer from COPD, Chronic Heart Disease, Stroke or are over 65 years old.  Please call reception to make an appointment for your vaccination.

The practice will be closed for staff training on Wednesday 12th December 2018 between 1.30pm and 3.30pm. Patients will not be able to make appointments, speak to a doctor, order/collect or order prescriptions. All calls will be redirected to our out of hours service. We apologise for any inconvenience this may cause. Please note the drop in blood clinic will continue to operate from 11am to1pm that day.

Online services for Low Moor Medical Practice users

People registered with our surgery can now book appointments and request repeat prescriptions at a time convenient to them, thanks to GP online services. These services enable people to book GP appointments and request repeat prescriptions via their computer, smartphone or tablet at a time that suits them rather than calling or visiting the surgery.

 
This approach can save people a phone call or journey to the surgery, and allow them to do what they need to when the surgery is closed. Patients living with a long term health condition can also benefit by having a greater involvement in their healthcare as they can access test results and keep track of their treatment.
 
All patient information is kept securely. This page on the surgery website www.lowmoormp.co.uk allows access to the information patients need.
 
This service works alongside traditional methods of accessing services, it does not replace them. By allowing people who want to make appointments and order prescriptions this way, it should become easier for others to get through on the phone to the surgery during busy periods.
 
If you want to register for GP online services you will need to fill out a form at the surgery and show two forms of ID one of which should have a photo (such as a UK passport or driving licence) and the other should have your address (such as a council tax bill). If you don’t have photo ID or anything with your address on it, it doesn’t mean you will not be able to use online services, our surgery staff may be able to help.

Medical Students

The practice is now a training practice for doctors in training and hosts third year medical students. The students work along side our regular GP's in the consulting rooms, and hold some surgeries of their own. All patients will be advised if the offered appointment is with a student, who is supervised by one of our regular doctors. If you are unhappy with this, you will be offered an alternative appointment, but we cannot guarantee it will be on the same day.
